Dyspnea   is   a   cardinal    manifestation   of   chronic   heart      

failure (CHF) , commonly    causeed    by   pulmonary   edema   

resulting   from   high   pulmonary   microvascular   pressure(Pmv)

Because   repeated   direct   measures   of   central   hemodynamics   

are   impractical,care   of   patients   with   CHF   involves   careful   

clinical   assessment   to   estimate   Pmv   However,   the   accuracy   

of   clinical   assessment   of   central   hemodynamics   is   limited   

Consequently , a   dynamic   biomarker   of   heart   failure   status ,

and   more   specifically   pulmonary   edema,   might   aid   in   the   

management   of   this   chronic , relapsing   condition .

      Brain   natriuretic   peptide   (BNP)   and   its   aminoterminal   

portion   (NT-proBNP)   are   secreted   from   the   ventricular   

myocardium   in   response   to   wall   stretch  ,   and   plasma   

concentrations   are   related   to   left   ventricular (LV)   filling   

pressure . However ,   these   biomarkers   do   not   reflect   the   

effects   of   hemodynamic   abnormalities   on   important   end   

organs , such   as   the   lungs .
